IDEXX Sample Statement

Below are samples of the two different sections you may see with your monthly IDEXX billing document. To understand different parts of the invoice, move your cursor over any item and a description of that item appears. These include the Statement Section and the Laboratory Service or Telemedicine Monthly Invoice Section. You may have multiple pages for each section. If you are not a customer of Laboratory Services (IDEXX Reference Laboratories) or IDEXX Telemedicine (CardioPet®, RadioPet®) or you did not have business with those services for that month, you will not receive those sections of the monthly billing documents.

You can choose to receive your monthly billing documents by mail, fax or e-mail. If you would like to change the method of delivery for your invoices and monthly billing documents, go to the method of delivery form and follow the instructions. You can make this change at any time.

This section provides the detail of each purchase during the month with each IDEXX Companion Animal Group business.

This section shows all individual tramsactions for the month with each IDEXX Companion Animal Group business. This includes individual invoices and charges, payments and credits based on transactions that occured during hte month. For detail of your charges for IDEXX Laboratory Services and/or IDEXX Telemedicine, please see that section below of the monthly billing document.

Laboratory Services/Telemedicine Monthly Invoice Detail Section
If you are a customer of IDEXX Reference Laboratories or IDEXX Telemedicine your monthly Invoice detail is included with your monthly billing document. These pages will show the detail for tests performed or consultations provided by IDEXX Refernce Laboratories or IDEXX Telemedicine for the month.

Laboratory Services/Telemedicine Invoice Section

The pages in this section provide the monthly invoice detail for IDEXX Laboratory Services and IDEXX Telemedicine. This includes the date of the test, the client/animal name and the cost of the test. The total of Laboratory Services and Telemedicine Services, if any, are included on the Consolidated Summary section described above.
